This information must include the brand of lens, base curve and lens power for the type of lens you are currently prescribed. This can be obtained from your current lens packaging, or the written specification provided by your optician. Unless specified by your optician, we are unable to change a prescribed brand on request.

You must also provide the date of your last contact lens consultation (not the date of your last sight test, unless the consultation took place at the same time).

In the fulfillment of an order and the provision by you of your current opticians contact details (including full postal address and telephone number), you provide us with the authority to contact them to check the validity and accuracy of the information given with respect to your occular health. Such authority is necessary to allow us to comply not only with our company terms and conditions, but also our duties as opticians and current legislation when in receipt of an order unaccompanied by a written contact lens specification.
